I know what you mean with the falling asleep !
I started meditation to find a cure for insomnia, and meditation calmed me down so much I kept falling asleep all the time.
I finally forced myself to sit in the Lotus position ( well, half Lotus, I am not a very flexible person :P ) and sitting up straight helped a lot.
Also, keeping your eyes half closed instead of entirely closed may help as well.
Be sure not to fix your sight on anything, try to "widen" your view to see everything instead of one thing.
And if nothing else helps, try to go to bed one hour earlyer every day, and use your meditation to fall asleep earlyer

keep doing that for a week and you will be so wonderfully rested that meditation sessions will become easyer
Oh, btw, meditation for pain management is a wonderfull thing, and I have heard of a "trick" that has brought much benefit for people with continuing pain:
Make them focus on the pain during meditation !
However, don't make them focus on pain in this way: Ooh that spot is very sore, gosh that hurts a lot, I am really suffering, man it is intolerable...
That would be wrong as it enhances the pain.
Instead, make them try to observe the pain "from a distance" see that the pain in the leg is only in the leg, not in the entire body, try to make them see the entire body and show where the pain is located as for instance a red light coming from the sore spot.
Now, make them accept the fact that the pain is there.
Fighting the pain makes the pain worse, accepting it fully makes the pain less.
Make them say in their mind during meditation: I accept this pain, it is there and I know that it will stay no matter what I do. Therefor I accept it. I smile at my pain, in a loving, kind way, like I would do to a small child.
I accept it fully as a part of my life.
this way, they will focus on their pain in the same way that they would focus on their breath or a mantra during meditation.
Suddenly the pain is much better to tolerate.
