Not necessarily. A person who feels perfectly ok can use positive affirmations and mindfulness (being present) to stay that way.
The other important point is to make sure you say it as if it exists already. As islovin said:
MY goal is to be healthy...
I want to be healthy.
One of the first things I say to myself in the present tense...
"I am physically fit".

Putting it in the present moment starts to make the unconscious mind believe it to be so (even if your conscious mind is telling you it's not true) and saying it several times over helps to reinforce that in the unconscious. The unconscious will then be more perceptive of the things around you that help to bring that statement to truth... they may just be small things but if you look out for them, you'll notice them and then you can act on them and help yourself to make it true. This is where a lot of people go wrong with LOA. They believe that simply stating it means it will come true and that they will have to do nothing themselves to make it so... whereas the statement simply helps our awareness to become more perceptive of the situations/objects/whatever in our lives that can help it become true.
If you try it and find it doesn't seem to be working there may be what, in EFT terms is called Psychological Reversal. This is where the Conscious and Unconscious truly believe different things. e.g. a common one I have found with people is where they consciously say "I want to lose weight" and consciously they really believe that. However, using muscle testing we find out that their unconscious is saying "I don't want to lose weight". Once this becomes apparent we can find out why the person doesn't want to lose weight, remove the pschological reversal* and deal with the issues that have been preventing them from wanting to lose weight.
* to remove psychological reversal, if you know EFT, (if you don't download the free manual from
EFT Home - World Center for EFT (Emotional Freedom Techniques) for the instructions), it can simply be a case of tapping on the karate chop point on the side of the hand whilst saying "Even though I don't want to lose weight (or whatever the issue is), I'm OK (or "I choose to be OK")"
So, using something like EFT can work really well with the Law of Attraction for overcoming issues and instilling positive self belief and attitude. Couple that with meditation and you've got one hell of a toolkit to use for self healing and getting your life back on track.
